ABSTRACT

ABSTRACT Objective: Identify the incidence and factors associated with reoperation due to bleeding in the postoperative of a cardiac surgery, in addition to the clinical outcomes of patients. Method: Prospective cohort study, conducted in an Intensive Care Unit (ICU), with adult patients undergoing cardiac surgery. Patients diagnosed with coagulopathies were excluded. The patients were followed up from hospitalization to hospital discharge. Results: A total of 682 patients were included, and the incidence of reoperation was 3.4%. The factors associated with reoperation were history of renal failure (p = 0.005), previous use of anticoagulant (p = 0.036), higher intraoperative heart rate (p = 0.015), need for transfusion of blood component during intraoperative (p = 0.040), and higher SAPS 3 score (p < 0.001). The outcomes associated with reoperation were stroke and cardiac arrest. Conclusão: Reoperation was an event associated with greater severity, organic dysfunction, and worse clinical outcomes, but there was no difference in mortality between the groups.


RESUMEN Objetivo: Identificar la incidencia y los factores asociados con la reintervención por sangrado en el postoperatorio de cirugía cardiaca, además de los resultados clínicos de los pacientes. Método: Estudio de cohorte prospectivo, realizado en una Unidad de Cuidados Intensivos, con pacientes adultos sometidos a cirugía cardiaca. Se excluyeron a los pacientes con diagnóstico de coagulopatías. Los pacientes tuvieron un seguimiento efectuado desde el ingreso hasta el alta hospitalaria. Resultados: Se incluyeron a 682 pacientes, y la incidencia de reintervención fue del 3,4%. Los factores asociados a la reintervención fueron antecedentes de insuficiencia renal (p = 0,005), uso previo de anticoagulante (p = 0,036), mayor frecuencia cardiaca intraoperatoria (p = 0,015), necesidad de transfusión de hemocomponentes en el intraoperatorio (p = 0,040) y mayor puntaje en SAPS 3 (p < 0,001). Los desenlaces asociados con la reintervención fueron accidente cerebrovascular y paro cardiorrespiratorio. Conclusión: La reintervención fue un evento asociado con mayor gravedad, disfunción orgánica y peores desenlaces clínicos, pero no hubo diferencia en la mortalidad entre los grupos.


RESUMO Objetivo: Identificar a incidência e os fatores associados à reoperação devido sangramento no pós-operatório de cirurgia cardíaca, além dos desfechos clínicos dos pacientes. Método: Estudo de coorte prospectivo, realizado em Unidade de Terapia Intensiva (UTI), com pacientes adultos submetidos à cirurgia cardíaca. Foram excluídos pacientes com diagnóstico de coagulopatias. Os pacientes foram acompanhados desde a internação até a saída hospitalar. Resultados: Foram incluídos 682 pacientes e a incidência de reoperação foi 3,4 %. Os fatores associados à reoperação foram: histórico de insuficiência renal (p = 0,005), uso prévio de anticoagulante (p = 0,036), maior frequência cardíaca intraoperatória (p = 0,015), necessidade de transfusão de hemocomponentes no intraoperatório (p = 0,040) e maior pontuação no SAPS 3 (p < 0,001). Os desfechos associados a reoperação foram: acidente vascular encefálico e parada cardiorrespiratória. Conclusão: A reoperação foi um evento associado a maior gravidade, disfunção orgânica, e piores desfechos clínicos, porém não houve diferença de mortalidade entre os grupos.

ABSTRACT

Diversos motivos inciden en que los pacientes se sometan a cirugía bariátrica para hacer frente a la obesidad. Pese a ser el procedimiento más eficaz contra el exceso de peso, existe una alta tasa de pacientes que después de la cirugía, recuperan un porcentaje importante del peso perdido. Esta re-ganancia de peso genera consecuencias médicas, físicas y psicológico-emocionales; y en muchos casos lleva a que los pacientes vuelvan a someterse a intervenciones quirúrgicas con el objetivo de reactivar la pérdida de peso. No obstante, las re-operaciones no suelen tener la misma eficacia en términos de la pérdida de peso y su mantención. Esta investigación se centra en los significados psicológicos de la experiencia de someterse a más de una intervención quirúrgica para perder peso, con el objetivo de comprender lo que implica este proceso para los pacientes, así como también las repercusiones que trae consigo someterse a más de una intervención de este tipo. La metodología que se utilizó fue cualitativa con un enfoque desde la Teoría Fundamentada y con un alcance descriptivo para comprender lo que deben vivir estos pacientes. Para llevar a cabo esta investigación, se realizaron entrevistas semi-estructuradas a cuatro mujeres y un hombre, quienes previamente se habían sometido a una intervención quirúrgica para perder peso, y luego de unos años y de haber recuperado el peso perdido se sometieron a una cirugía bariátrica. La elección de la segunda intervención se asoció a la creencia que la recuperación de peso se debió principalmente a la técnica de la primera intervención, evaluación que cambia hacia una percepción de bajo control en la alimentación, tras la re-ganancia de peso post-segunda intervención. Se observa niveles importantes de frustración tras esta segunda re-ganancia.


There are several reasons why patients choose to undergo bariatric surgery to cope with obesity. Despite being the most effective procedure against excess weight, there is a high rate of patients who, after surgery, recover a significant percentage of the weight lost. This regain of weight generates medical, physical and psychological-emotional consequences; and in many cases it leads to patients re-undergo surgery with the aim of reactivating weight loss. However, re-operations are usually not as effective in terms of weight loss and weight maintenance. This research focuses on the psychological meanings of a patient's experience of more than one surgical intervention to lose weight, with the aim of understanding what this process implies for patients, as well as the repercussions of obtaining more than one intervention of this type. The specified methodology was qualitative with an approach from the Grounded Theory and with a descriptive scope to understand what these patients must live. To carry out this research, semi-structured interviews were conducted with four women and one man, who had previously undergone surgery to lose weight, and after a few years and having recovered the lost weight, underwent surgery. bariatric The choice of the second intervention was associated with the belief that weight recovery was mainly due to the technique of the first intervention, an evaluation that changes towards a perception of low control in the diet, after the post-weight gain second intervention. ABSTRACT

Abstract Introduction Cholesteatomas are benign tumors consisting of skin, and growing inside a retraction pocket in the tympanic membrane. Cholesteatomas can occupy the entirety of the middle ear, and are known for their osteolytic capabilities. Surgery is the only curative treatment for cholesteatomas. Objective To describe the risk of recurrence after first-time surgically-treated middle- ear cholesteatoma (STMEC1) on the island of Funen from 1983 to 2015. Methods Cases of STMEC1 were identified in the Danish National Hospital Register. The medical records were reviewed. Time-to-event analyses were applied. The ears were followed from STMEC1 to a secondary cholesteatoma, emigration, death, or end of follow-up. Results Records from 1,006 patients with STMEC1 were reviewed. A total of 54 patients were submitted to surgery on both ears. The total sample consisted of 1,060 ears with STMEC1; 300 were children's (< 16 years) ears, and 760 were adult's ears. The total observation time was of 12,049 years. The overall estimated proportion with recurrence 5 years after surgery was of 37% in children and of 15% in adults. The older the child was at the first surgery, the risk decreased by 7% per year. In children, canal wall up (CWU) mastoidectomy without obliteration was associated with a hazard ratio for recurrence of 1.9 (95% confidence interval [95%CI]: 1.2-3.0) compared with CWU with obliteration. Conclusion Compared with adults, children were had 2.6 times more risk of recurrence. Procedures performed without mastoidectomy had the lowest risk of recurrence. In children, obliteration was associated with a significantly lower risk of recurrence. However, patients were not randomized regarding the surgical approach; thus, the association between approach and risk of recurrence was likely influenced by confounding factors.

ABSTRACT

Introducción La mastectomía es el tratamiento estándar de la recidiva local (rl) luego de un tratamiento conservador por cáncer de mama. Para algunos centros, una segunda cirugía conservadora es una opción para un subgrupo de pacientes. Objetivos Identificar alguna variable que nos permita realizar una segunda cirugía conservadora ante una recidiva local en paciente con cáncer de mama. Material y método Estudio retrospectivo de 1.661 pacientes con cáncer de mama estadio temprano sometidas a un tratamiento conservador. Se diagnosticaron 75 recurrencias locales. En 33 casos se les realizó una segunda cirugía conservadora. Se evaluaron características clínicas, histológicas y moleculares de la recurrencia local. Resultados La edad promedio al momento de la recaída fue de 55,4 años. El tamaño tumoral promedio de la rl fue de 12 mm. Los receptores hormonales fueron positivos en el 69,8%. Durante un seguimiento promedio de 7,6 años, 9 (27,1%) pacientes presentaron una segunda recurrencia local y 5 (15,1%) murieron por enfermedad metastásica. La sobrevida global fue del 84,8%. El tiempo de intervalo promedio en el grupo libre de enfermedad fue de 103,3 meses, y el 70% de las pacientes recurrieron después de los 60 meses. En el grupo de pacientes con progresión, el 38,4% presentó la recurrencia después de los 60 meses con un intervalo promedio de 57,2 meses. Conclusiones Según nuestros resultados, el tiempo que transcurre hasta la recurrencia local es una variable a tener en cuenta para seleccionar pacientes para una segunda cirugía conservadora. Las pacientes con recurrencias posteriores a 60 meses presentaron mejor evolución.


Introduction The standard surgical treatment after breast conservative treatment (bct) for an ipsilateral breast tumor recurrence (ibtr) is salvage mastectomy. For some centers, second conservative surgery is an option for a subset of patients. Objectives Identify some variable that allows us to perform a second conservative surgery before a local recurrence in a patient with breast cancer. Materials and method Retrospective study of 1,661 patients with early stage breast cancer undergoing conservative treatment. 75 ibrt were diagnosed. In 33 cases, a second conservative surgery was performed. Clinical, histological and molecular characteristics of the ibrt were evaluated. Results Average age of 55.4 years at the time of relapse. The mean tumor size of the rl was 12 mm. The hormone receptors were positive in 69.8%. During an average follow-up of 7.6 years, 9 (27.1%) patients had a second ibrt and 5 (15.1%) died of metastatic disease. Overall survival was 84.8%. The mean interval time in the disease free group was 103.3 months, and 70% of the patients recurred after 60 months. In the group of patients with progression, 38.4% presented recurrence after 60 months with an average interval of 57, 2 months. Conclusions According to our results, the time to local recurrence is a variable to be taken into account in selecting patients for conservative management. Patients with recurrences after 60 months presented better evolution.

BACKGROUND/AIMS: This stuy evaluated the role of a second-look endoscopy after gastric endoscopic submucosal dissection in patients without signs of bleeding. METHODS: Between March 2011 and March 2012, 407 patients with gastric neoplasms who underwent endoscopic submucosal dissection for 445 lesions were retrospectively reviewed. After the patients had undergone endoscopic submucosal dissection, they were allocated to two groups (with or without second-look endoscopy) according to the following endoscopy. The postoperative bleeding risk of the lesions was not considered when allocating the patients. RESULTS: The delayed postoperative bleeding rates did not differ between the two groups (with vs without second-look endoscopy, 3.0% vs 2.1%; p=0.546). However, a tumor in the upper-third of the stomach (odds ratio [OR], 5.353; 95% confidence interval [CI], 1.075 to 26.650) and specimen size greater than 40 mm (OR, 4.794; 95% CI, 1.307 to 17.588) were both independent risk factors for delayed postoperative bleeding. Additionally, second-look endoscopy was not related to reduced delayed postoperative bleeding. However, delayed postoperative bleeding in the patients who did not undergo a second-look endoscopy occurred significantly earlier than that in patients who underwent a second-look endoscopy (4.5 and 14.0 days, respectively, p=0.022). CONCLUSIONS: A routine second-look endoscopy after gastric endoscopic submucosal dissection is not necessary for all patients.

BACKGROUND/AIMS: Second-look endoscopy is performed to check for the possibility of post-endoscopic submucosal dissection (ESD) bleeding and to perform prophylactic hemostasis in most hospitals; however, there is little evidence about the efficacy of second-look endoscopy. We investigated whether second-look endoscopy after ESD is useful in the prevention of post-ESD bleeding. METHODS: A total of 550 lesions with gastric epithelial neoplasms in 502 patients (372 men and 130 women) were treated with ESD between August 18, 2009 and August 18, 2010. After the exclusion of three lesions of post-ESD bleeding within 24 hours, 547 lesions (335 early gastric cancers and 212 gastric adenomas) were included for the final analysis. RESULTS: The occurrence rate of delayed post-ESD bleeding was not significantly different between the second-look group and the no second-look group (1% vs 2.5%, p>0.05). The only predictor of delayed bleeding was tumor size, regardless of second-look endoscopy after ESD (22.8+/-9.87 vs 15.1+/-10.47, p<0.05). There was no difference between the prophylactic hemostasis and nonprophylactic hemostasis groups, including the occurrence rate of delayed bleeding. In the second-look group with prophylactic hemostasis, the hospital stay was more prolonged than in the second-look group without prophylactic hemostasis, but there was no significant difference (p=0.08). CONCLUSIONS: Second-look endoscopy to prevent delayed bleeding after ESD provides no significant medical benefits.

ABSTRACT

BACKGROUND: Meniscus tears are commonly associated with anterior cruciate ligament (ACL) ruptures. It is essential to repair meniscal tears as much as possible to prevent early osteoarthritis and to gain additional stability in the knee joint. We evaluated the results of arthroscopic all-inside repair using the Meniscal Viper Repair System (Arthrex) on meniscus tears simultaneously with ACL reconstruction. METHODS: Nineteen out of 22 patients who were treated with arthroscopic all-inside repair using the Meniscal Viper Repair System for meniscus tear associated with ACL rupture were evaluated. ACL reconstructions were performed at the same period. The mean follow-up period was 16.5 months (range, 12 to 24 months). The clinical results of the meniscus repair were evaluated by symptoms (such as catching or locking), tenderness, effusion, range of motion limitation, and the McMurray test. Clinical success was defined by negative results in all five categories. The Hospital for Special Surgery (HSS) score was evaluated. Objective results were evaluated with secondary look arthroscopy or magnetic resonance imaging (MRI). The MRI results were categorized as completely repaired, incompletely repaired, and failure by Henning's classification. The results of second-look arthroscopy were evaluated with the criteria of meniscal healing. RESULTS: The clinical success rate was 95.4% and the HSS scores were 93.9 +/- 5.4 at the final follow-up. According to Henning's classification, 15 out of 18 cases showed complete healing (83.3%) and two cases (11.1%) showed incomplete healing. Seventeen out of 18 cases that underwent second-look arthroscopy showed complete healing (94.4%) according to the criteria of meniscal healing. Only one case showed failure and the failure was due to a re-rupture at the sutured area. Complications of ACL reconstruction or meniscus repair were not present. CONCLUSIONS: The results demonstrate that arthroscopic all-inside repair using the Meniscal Viper Repair System is an effective treatment method when it is performed simultaneously with ACL reconstruction.

BACKGROUND/AIMS: Many authors recommend performing a second-look endoscopy (SLE) to reduce the frequency of delayed bleeding after endoscopic submucosal dissection (ESD) for gastric neoplasms, but these recommendations have been made despite a lack of reliable evidence supporting the effectiveness of SLE. METHODS: From January 2012 to May 2013, we investigated 441 gastric neoplasms treated by ESD to assess the risk factors for delayed bleeding. Delayed bleeding occurred in four of these lesions within 1 postoperation day. Therefore, we enrolled the patients with the remaining 437 lesions to determine the utility of SLE performed on the morning of postoperative day 2. All lesions were randomly assigned to SLE (220 lesions) groups or non-SLE (217 lesions) groups. RESULTS: Delayed bleeding occurred in 18 lesions (4.1%). A large tumor size (>20 mm) was the only independent risk factor for delayed bleeding (p=0.007). The chance of delayed bleeding was not significantly different between the patients receiving a SLE (eight cases) and those patients not receiving a SLE (six cases, p=0.787). Furthermore, SLE for lesions with a large tumor size did not significantly decrease delayed bleeding (p=0.670). CONCLUSIONS: SLE had little or no influence on the prevention of delayed bleeding, irrespective of the risk factors.

ABSTRACT

Tuberculous otitis media (TOM) is rare in ENT department, and is frequently misdiagnosed as otitis media. Thus early systemic treatment is very important for TOM. We reported a case report with TOM to highlight development of the disease and difficulties in clinical treatment in late stage of TOM. Implantation of ossified and eroded cochlea poses many unique challenges to both the surgeon and programming team. With thorough preparation and complete knowledge about characters of specific issues, implantation would be performed successfully, and patients with ossified cochlear could benefit from cochlear implantation.

ABSTRACT

<p><b>BACKGROUND</b>Several reports have shown the progression of articular cartilage degeneration after anterior cruciate ligament (ACL) reconstruction. No report has been published about the cartilage comparing changes after single-bundle (SB) and double-bundle (DB) ACL reconstructions. The purpose of this study was to evaluate the articular cartilage changes after SB and DB ACL reconstructions by second-look arthroscopy.</p><p><b>METHODS</b>Ninety-nine patients who received arthroscopic ACL reconstruction were retrospectively reviewed at an average of 14 months after reconstruction, 58 patients underwent SB ACL reconstruction and 41 patients underwent DB ACL reconstruction. Hamstring tendon autografts were used in all patients. Second-look arthroscopy was done in conjunction with the tibial staple fixation removal at least one year after the initial ACL reconstruction. Arthroscopic evaluation and grading of the articular cartilage degeneration for all patients were performed at the initial ACL reconstruction, and at the second-look arthroscopy.</p><p><b>RESULTS</b>The average cartilage degeneration at the patellofemoral joint (PFJ) was found significantly worsened after both SB and DB ACL reconstructions. This worsening were not seen at medial tibiofemoral joint (TFJ) and lateral TFJ. Grade II cartilage damage was the most common. At second-look arthroscopy, the average patellar cartilage degeneration was 1.14 ± 0.14 (at first look 0.52 ± 0.11) for the SB group, and 1.22 ± 0.15 (at first look 0.56 ± 0.12) for the DB group. The average trochlear cartilage degeneration was 1.05 ± 0.16 (at fist look 0.10 ± 0.06) and 0.66 ± 0.17 (at fist look 0.17 ± 0.09), respectively. The average patellar cartilage degeneration showed no significant difference in both groups. However, the average trochlea cartilage degeneration in DB group was significantly less than in SB group.</p><p><b>CONCLUSIONS</b>Patellofemoral cartilage degeneration continued to aggravate after ACL reconstruction. DB ACL reconstruction could significantly decrease the trochlea cartilage degeneration compared with SB ACL reconstruction.</p>

ABSTRACT

OBJETIVO: Avaliar a possibilidade de perda de extensão do joelho, após artroplastias de revisão, realizadas com acessos do tipo quadriceps snip e osteotomia do tubérculo anterior da tíbia (TAT), comparativamente, entre ambos e com um grupo controle, em que foi utilizado o acesso parapatelar medial convencional. MÉTODO: Este estudo foi aprovado pelo Comitê de Ética em Pesquisa do INTO. A perda de extensão do joelho foi avaliada em pacientes submetidos a artroplastias de revisão, unilaterais, realizadas pelo Grupo de Joelho do Instituto Nacional de Traumatologia e Ortopedia - INTO, em que foram utilizados um dos seguintes acessos: convencional - parapatelar medial, snip do quadríceps ou osteotomia do tubérculo anterior da tíbia. A amostra foi composta por 26 indivíduos, cuja idade variou entre 52 e 80 anos, distribuídos em três grupos: grupo I: acesso parapatelar medial convencional (n = 10); grupo II: quadriceps snip (n = 8); e grupo III: osteotomia do TAT (n = 8). O tempo de seguimento médio foi de 18,57 meses. A perda da extensão do joelho foi determinada pela diferença entre o grau de extensão ativa pré e pós-operatória. Na avaliação estatística, foi utilizado o teste de análise de variância ANOVA, de fator único, com o nível de significância a = 0,05 (5 por cento). RESULTADOS: A perda média de extensão da série foi igual a 8,57º, variando entre 2º e 30º. O p-valor obtido foi igual a 0,112, o que permite assumir a hipótese de igualdade entre os grupos. CONCLUSÃO: No presente estudo, não foi observada diferença significativa entre os grupos compostos pelos três tipos de acesso utilizados para as revisões de artroplastias, no que tange ao parâmetro perda de extensão.


OBJECTIVE: To assess the possibility of extension loss of the knee after revision arthroplasties performed with approaches like quadriceps snip and TAT osteotomy, comparing the two methods and using a control group in who the conventional medial parapatellar approach was used. METHOD: This study was approved by the INTO Research Ethics Committee. The loss of extension of the knee was evaluated in patients submitted to unilateral revision arthroplasties of the knee conducted by the Knee Group of the INTO - National Institute of Traumatology and Orthopedics, the following approaches having been used: conventional - medial parapatellar, quadriceps snip, or anterior tibial tubercle osteotomy. The series included 26 individuals, ages ranging from 52 to 80, distributed into three groups: Group I: conventional - medial parapatellar approach (n = 10); Group II: quadriceps snip (n = 8); and Group III: TAT osteotomy (n = 8). Mean follow-up was 18.57 months. Extension loss of the knee was determined by the difference between the range of active extension before and after surgery. In the statistical assessment, the ANOVA variance analysis test was used, with a single factor, with significance level a = 0,05 (5 percent). RESULTS: Mean extension loss in the series was equal to 8.57º, varying between 2º and 30º. The p-value obtained was equal to 0.112, which leads to presume the hypothesis that groups are equal to one another. CONCLUSION: No significant difference was observed among the groups made up by the three types of approach used for arthroplasty revisions in terms of the loss of extension parameter.

ABSTRACT

O objetivo foi estudar as alterações da força de fixação dos parafusos pediculares nos corpos vertebrais ao variar a angulação durante o reposicionamento. Foram utilizados 8 suínos, totalizando 40 vértebras lombares. Dividimos em 4 grupos de estudo. No primeiro grupo os parafusos foram colocados a 0º (zero graus), no segundo introduzidos, retirados e recolocados na mesma posição (zero graus), no terceiro houve uma variação da angulação de 14º no reposicionamento e no quarto uma variação da angulação inicial de 28 graus. Todos os parafusos foram testados quanto à força de arrancamento. No resultado o grupo I apresentou uma média de arrancamento de 146,85N; o grupo II uma média de arrancamento de 77,34N. O grupo III 97,75N. O grupo IV teve uma média de 110,02N. Isso mostrou a necessidade de evitar o reposicionamento dos parafusos devido a perda de força de fixação no corpo vertebral. Quando reposicionados recolocá-los em angulações as mais diferentes possíveis da inicial, comprometendo assim, o mínimo possível a fixação.


The objective of this paper was to study pedicular screws fixation strength on vertebral bodies by varying its repositioning angle. For this Study, eight pigs were selected, making up a total of 40 lumbar vertebrae, which were divided into 4 study groups. In the first group, the screws were inserted at zero degree, and in the second group, they were inserted and reinserted at the same position, while in the third group, the screws were reinserted within a 14-degree range, and, in the fourth group, within a 28-degree range. All screws were tested for pullout strength. Group I presented a pullout average of 146.85N. Group II presented a pullout average of 77.34N. Group III had an average of 97.75N. Group IV had an average of 110.02. It was concluded that the procedure of replacing screws should be avoided. Should any repositioning is required, this should be performed by changing repositioning angles as much far from the first one as possible.

ABSTRACT

The role of a routine second transurethral resection in evaluating and management of patients with bladder tumor is defined. From January 2004 to October 2006, 100 patients with new or recurrent bladder tumors underwent repeat transurethral resection within 4-6 weeks after the initial resection, and the results, including the presence of residual tumor and tumor stage, were compared. Of 100 cases, 28 [28%] had no tumor and 72[72%] had residual tumor on repeat TUR. Out of 64 cases with superficial [Ta, Tis, T1] bladder tumors, 20[31%] had residual non-invasive tumor and 13 [20%] were upstaged to muscle invasive tumor. Among 36 patients with a muscle invasive tumor, 10[28%] had no residual tumor on re-TURBT. Results of second resection had changed treatment option in 39 patients [39%] from the initial treatment recommendation which was given after the first TUR. Many patients with bladder tumor have residual tumor present after an initial TUR. Routine repeat resection is advised to control noninvasive tumors and to detect residual tumor invasion

ABSTRACT

Objetivo: Avaliar clínica e radiograficamente a revisão femoral das artroplastias totais de quadril com haste de Wagner, demonstrando que o implante obteve ótimos resultados, sem necessidade de enxertia óssea. Métodos: No período de janeiro de 1997 e janeiro de 2004, no Instituto Nacional de Traumato-Ortopedia, foram realizadas 197 revisães femorais com haste de Wagner, com tempo de evolução superior a 12 meses. A principal indicação da cirurgia de revisão foi o afrouxamento asséptico do componente femoral em 147 casos (74,6%). Osteotomia trocanteriana estendida foi utilizada em 146 revisães (74%). Resultados: Os pacientes foram analisados clinicamente de acordo com os critérios de Merle DÆAubigné e Postel, modificados por Charnley, e, radiograficamente, avaliando-se variáveis como afundamento do implante, remodelação óssea proximal, consolidação da osteotomia (quando utilizada) e incorporação do enxerto. Conclusão: A haste femoral de Wagner é um implante de fixação estável, que confere boa remodelação óssea, sem a necessidade de utilização de enxertia óssea, observada em 96% dos casos. Clinicamente, 86,3% dos casos apresentaram resultados bons e excelentes.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the retroperitoneal lymph node metastasis in epithelial cancer of the ovary and offer scientific indications for lymph node radical dissection.</p><p><b>METHODS</b>Fifty-eight patients with ovarian cancer treated from January 1990 to December 2000 were retrospectively reviewed. Single-factor and multifactor analysis with Logistic regression model were performed by SPSS 10.0 statistic software.</p><p><b>RESULTS</b>The metastasis rates of overall lymph nodes, pelvic nodes and para-aortic nodes were 48.3%, 37.9% and 25.9% respectively, among which no significant difference was noted (P > 0.05). Single-factor analysis showed that tumor location, ascitic condition, clinical stage and the size of residual tumor were associated with retroperitoneal lymph node metastasis. Multifactor analysis revealed that clinical stage and size of residual tumor were independent risk factors for metastasis of retroperitoneal lymph nodes.</p><p><b>CONCLUSION</b>For early ovarian cancer patients, it is extremely important to perform radical dissection of the retroperitoneal lymph nodes. For advanced or residual lesions, radical dissection of pelvic nodes and para-aortic nodes could be considered in the second exploration.</p>

ABSTRACT

OBJECTIVE: To determine survival among patients with epithelial ovarian carcinoma (EOC) who underwent a second-look laparotomy (SLL) and those refusing the procedure. Also to analyze factor(s) influencing the survival of the patients. METHOD AND MATERIAL: Medical records were reviewed of patients with advanced EOC who were clinically free of disease after primary surgery and platinum-based chemotherapy between January 1, 1992, and December 31, 1998. All of them were offered SLL. Measurement outcomes include patient survival and disease-free survival. RESULTS: There were 50 patients with clinically complete remission after chemotherapy. Sixteen patients underwent SLL, and thirty-four patients refused the procedure (NSLL). Seven patients (43.8%) were reported to have positive SLL. After the median follow-up time of 35 months, 12 patients had died, and 5 patients were lost to follow-up. The median survival time for patients with SLL was about 60 months. Five-year survival rates of patients in the SLL, and NSLL groups were 37 per cent (95%CI = 7%-69%), and 88 per cent (95%CI = 65%-96%) respectively (P<0.001). The median time to relapse was about 25 months for patients with negative SLL. Five-year disease-free survival rates of patients in the negative SLL, and NSLL groups were 28 per cent (95%CI = 4%-59%), and 54 per cent (95%CI = 34%-70%) respectively (P=0.251). By Cox regression analysis, tumor grade was the only significant prognostic factor influencing patients' survival (HR = 6, 95%CI of HR = 1.2-34.2). CONCLUSION: The second-look laparotomy doesn't have a favorable impact on overall and disease-free survival. Tumor grade is the only independent prognostic variable for survival of the patients.
